Student Scholarship Claim Form

SFPE Scholarships are paid directly to the institution of the student’s choice. The information collected on the claim form is required so that the student’s account is credited accurately. A copy of the college acceptance or admissions letter and class schedule are required.

Please submit the form at least 20 days before the institutions’ class start date to allow time for funds to be disbursed by SFPE and received and applied by the institution.
